The online PQQ is a shortened version authorised by Concept. It removes repeated initials and detailed medical screening while retaining the source IMS and essential declarations. Read the full IMS before signing the declaration. The source document remains available throughout.
No contractor is authorised to begin solely because they have submitted a form. Concept must review and approve the PQQ, then complete the relevant work instructions and site arrangements.
Payment onboarding remains incomplete until the required documentation and review are complete. This process does not remove contractual or statutory payment rights.

Completed PQQs and insurance certificates are intended for delivery to Daryl@conceptgl.co.uk. Only include information relevant to safe performance, emergency response or adjustments. Do not upload medical records or detailed diagnoses unless specifically requested through an appropriate confidential process.
